Forms help
Submit the form according to type:
- PDF: Print the form, then complete and submit it according to instructions found on the form.
- E-Form: E-Forms open in a new browser window. Complete the form, then click Submit. The form is automatically routed to a campus department or office (or multiple offices) for approval. You'll receive an e-mail message when your form has been submitted to and approved by all necessary parties
